ADMISSION OF LEARNERS:

Parents who wish to enroll their sons/daughters in PISD must meet/submit the following requirements:

​

1. Old Learners

    • Form 138 (Report Card)

    • Filled-out Personal Information Sheet

    • Guidance Form

​

2. New Learners        

    a. Examinations            

       An applicant for enrollment must pass the following:          

      • Oral Interview          

      • Written Entrance Examination        

​

    b. Credentials needed          

      • Form 138-E          

      • Filled-out Personal Information Sheet          

      • Guidance Form          

      • 2 pcs. 1x1 sized picture with white background          

      • Certificate of Good Moral Character          

      • NSO-Certified Birth Certificate        

​

    c. And any other document the school may deem necessary for admission

 

3. Foreigners

Foreigners must present their Special Permit to Study from the Bureau of Immigration and Deportation, Alien Certificate of Registration (ACR), passport, and must comply a & b under New Learners.
